The City of Palm Bay is one of over 8,000 designated passport acceptance facilities located throughout the United States.  Applications are accepted Monday through Friday from 8:30 a.m. to 4:30 p.m. at the Customer Service counter located in the lobby of City Hall, 120 Malabar Road SE. Once applications are accepted at the City of Palm Bay facility, they are submitted to one of thirteen regional passport agencies for processing and issuance of the passport.

Applications and other passport forms may be picked up in advance at our office, 120 Malabar Road SE between the hours of 8:30 a.m. and 5:00 p.m., Monday through Friday.

Applications must be signed in the presence of a customer service representative at our office when an application is filed. These forms are also available on the U.S. Department of States website.

New Application

To obtain a passport for the first time, the applicant will need to appear before an Acceptance Agent.  You will need the application (form DS-11),  two official passport photographs, proof of U.S. citizenship, and a valid form of photo identification such as a driver’s license.

You must make application in person at an Acceptance Facility if you are applying for a U.S. passport for the first time; if your passport is not in your possession; if your previous passport has expired and was issued more than 15 years ago; if your previous passport was issued when you were under age 16; or if your currently valid U.S. passport has been lost or stolen.

Passports are renewed by the individual via mail using form DS-82 when: 

Your most recent U.S. passport book and/or U.S. passport card is available to submit; your most recent U.S. passport book and/or U.S. passport card for which you are renewing has not been lost, stolen mutilated or damaged; you were issued your most recent U.S. passport book and/or U.S. passport card within the past 15 years; you were at least age 16 when your most recent U.S. passport book or U.S. passport card was issued; you still have the same name, or can provide legal documentation to support the name change.

If your passport has been altered or damaged, you cannot apply by mail; you must apply in person.

* The $35 Execution (processing) Fee is not charged when adults 16 and over with fully valid passport books apply for the passport card using a DS-82 form

Separate payments to the City of Palm Bay and to the US Department of State are required.

  • Passport Fee payable to: U.S. Dept of State - personal check, money order or certified check ONLY
  • Execution Fee payable to: City of Palm Bay - personal check, money order, cash or credit card. As a service to our customers, Express Postage may be purchased at City Hall and included with the payment made to the City of Palm Bay.  Additional fees apply for expedite service.
